This is an example of a Certificate of Insurance for Minnesota.

This form is often called:

--SR-22 (incorrect, but this is the common name in many other states)
--State of Financial Responsibility
--FR Filing

This form is proof of liability insurance for the vehicles listed.

There is no way to tell if the form is expired without an accompanying insurance I.D. card
or declarations page.

This form is NOT the "proof of insurance" listed in DPS regulations, rather it is the
"Certificate of Insurance" that is referenced in DPS 7409.3800.

A "Certificate of Insurance" is a more stringent requirement than "proof of insurance".
